Support our life-saving work through your donor advised fund

A grant from your donor advised fund (DAF) will help to support health care in Longmont and the surrounding communities during these ever-changing times.

A DAF, which is like a charitable savings account, gives you the flexibility to recommend how much and how often money is granted to Longs Peak Hospital Foundation. You can recommend a grant or recurring grants now to make an immediate impact, or you can use your fund as a tool for ongoing and future charitable gifts.

How to make a donation from your DAF
Call your fund manager and request a grant be made to Longs Peak Hospital Foundation, a qualified 501(c)(3) nonprofit organization, located at 1750 E. Ken Pratt Blvd., Longmont, CO 80504 (Tax ID: 82-1613059).

Your fund manager will send a check directly to Longs Peak Hospital Foundation from your DAF.

Please request that your name and address be included. We recommend that you contact Longs Peak Hospital Foundation to inform us of your gift, so we can thank you for your generosity.
